WHERE IS THE SERIES AND NUMBER ON THE PLASTIC VERSION OF THE DOCUMENT?
The medical policy of the new form differs significantly from the old document. Modern politics has two options: plastic and paper. As a rule, a plastic document is issued in addition to paper, and the information contained in both types of policies is identical. A plastic card is more practical in daily use, it does not crumple or crumple.
The form of the OMS insurance certificate has changed several times in recent years. Currently, this is regulated by the Order of the FFOMS of the Russian Federation dated August 15, 2000 “On the introduction of a unified system for forming the number of compulsory medical insurance health insurance policies”. Each digit in the document number has its own meaning, which is important for medical organizations and institutions that conduct accounting and reporting activities for health insurance organizations, the territorial and the Federal Fund for Mandatory Medical Insurance. Let’s say a little more about the information encoded in the 16 positions of the OMC policy number of the sample issue until 2012:
- 1 and 2 – indicate the code of the region in which this document was issued;
- 3 digits – double number, i.e. if the document is issued for the first time, then this digit is “0”. When reissued, this position varies from “1” to “9”;
- 4-7 positions – this is the year of birth of the insured citizen;
- 8 and 9 – indicate the month when the policyholder was born;
- 10th and 11th – birthday. This combination has its own characteristics: if the insured person is male, then “50” is added to the birthday number. If some information about the date of birth is not known, then zeros are written instead of numbers. For example, for a man born on December 10, 1991, the numbers 4 to 11 will be: 19911260. For a woman born in May 1938 (the number is unknown), the numbers 4-11 in the policy will be: 19380500;
- 12-15 positions – serial number assigned to people who have the same date of birth. It can be a number from “1” to “9999”.
- 16 digits, the last one is control. It is calculated using a special international calculation method.
It is necessary to pay attention to the fact that the policy issued after December 2011 does not have a similar decoding of the sequence of digital numbers. Digital series can be memorized or written down to name these numbers as needed. And they may be needed in a clinic or hospital, if the document itself is not available.

Details of the OMS certificate of insurance of the old form are on the front of the form (it is the only one) – they include the 16 digits printed in the line “Series / No.” The first 6 positions indicate the series of the document, and the next 10 – its number. The main differences in specifying the details of the old and new forms:
- The OMS policy number on the new sample forms on plastic and paper is located at the bottom of the front of the document and contains 16 digits;
- The old style document series and number are located in the upper third of the document and represent a digital series of 6 series positions and 10 number positions.
