How to Replace Lost TIN ID
If you have damaged or lost your TIN ID, don’t panic. You can request a replacement card from the BIR.
Lost TIN Identification Requirements
Simply prepare the following requirements to replace a lost or damaged TIN ID:
- Completed BIR Form 1905 (Registration Information Update Form) and BIR Form 0605 (Payment Form)
- For Lost TIN Identification: Notarized Affidavit of Loss
- For ID with damaged TIN: Old ID card and a photocopy
- Marriage certificate and old TIN card (if applicable)
Steps to Replace Lost or Damaged Tin ID
Once your requirements are complete, take them to your RDO. You can download the necessary forms or obtain copies of the RDO.
- Fill out Form BIR 1905. Don’t forget to put an X in the “Replacement Taxpayer Identification Number (TIN) Card” in Part II.
- Under “Reason/Details”, place an X in the “Missing/Damaged” box.
- Go to the Certificate of Registration (COR) Update window and submit your completed BIR Form 1905 and other requirements.
- Fill out Form BIR 0605 and have it verified at the payment window.
- Go to your RDO agent bank to pay for your lost or damaged card. The replacement fee usually costs around PHP 150 to PHP 300. Please request proof of BIR payment before paying the fee. The bank will verify your Form 0605 machine.
- After payment, return to the RDO and submit your machine verified Form 0605 and receipt to the COR update window.
- Wait for your new card to be issued.
